Extension of the Greater Choice for At Home Palliative Care Measure
Department of Health and Aged Care
Overview
In the 2017-18 Budget, the Commonwealth Government provided over $8.3 million for 3 years (2017-18 to 2019-20), to 11 Primary Health Network (PHN) pilot sites to develop innovative and targeted programs, at the local level, to improve access and coordination of palliative care and end of life care services in the home. The Commonwealth Government is now offering a closed, non-competitive grant of $300,000 over 12 months to each of these 11 original PHN pilot sites. The purpose of this grant is to extend the original Measure, allowing each of the pilot sites to continue exiting activity, build on investment and further embed initiatives to improve access to palliative and end-of-life care at the local level.
