GO3011 - Health Care Homes - Trial Support Funding

Overview

This grant opportunity will continue funding to the ten Primary Health Networks (PHNs) currently participating in the Health Care Homes (HCH) Program to employ dedicated practice facilitators to support the implementation of the HCH model under the HCH Trial Support Activity. A total of $3.3 million (GST exclusive) is available over two years from 2019-20 to 2020-21. The grant opportunity will align the funding provided to PHNs with the number of HCH practices they were supporting as at July 2019. Funding under the grant opportunity will cease on 30 June 2021.
