Overview

This grant opportunity contributes to the achievement of the Public Health and Chronic Disease Program under Outcome 1 – Health Access and Support Services, Program 1.5: Preventive Health and Chronic Disease Support. The objectives of the grant opportunity are to: cover the initial establishment and operating phase costs for the Facility including staffing, training, consumables, resources, and administration costs; and cover the cost of developing a sustainability plan for the Facility service to continue beyond the initial establishment and operating phase. The intended outcome of the grant opportunity is for increased care and support for adults between 18-60 years with terminal or chronic condition(s) in the Queanbeyan community and surrounding region.
