Overview

This grant opportunity will support the continuation and consolidation of two existing outreach programs – Medical Outreach Indigenous Chronic Disease Programme (MOICDP) and Healthy Ears – Better Hearing, Better Listening – into a single First Nations Chronic Disease Outreach Programme. The First Nations Health Outreach Services grant opportunity aims to improve efficiency and provide greater flexibility for jurisdictional fund holders to deliver multidisciplinary outreach services that address a broader range of chronic diseases, including hearing health.
