Overview

This grant opportunity provides funding to a select number of Primary Health Networks (PHNs) to support the establishment of mental health multidisciplinary services in primary care settings. The grant opportunity will facilitate the employment of health practitioners to optimise support for consumers with severe and/or complex mental health needs.  PHNs will have flexibility to choose the most appropriate funding model and model of care to suit local needs and market conditions. Services delivered would be tailored to individual needs, and could include care coordination and navigation, case management, brief interventions, psychosocial support and social prescribing. Funding for this grant opportunity will be available from 2025-26 to 30 June 2028. It is expected that grant funding will be valued at between $1.3 - $3.1 million annually, per PHN. NOTE: Please refer to the most recent version of the Frequently Asked Questions Version 3 and Addendum 3 published on 27 June 2025.
