Overview

The purpose of the grant opportunity is to improve access to eye care for First Nations patients through improved coordination of services, including improving linkages between the range of eye health services required by patients and delivery of those services. This will allow for coordination of services across the full spectrum of eye care, including those delivered through the Medical Outreach Indigenous Chronic Disease Programme (MOICDP), Rural Health Outreach Fund (RHOF), the Visiting Optometrists Scheme (VOS) and the eye element of the Eye and Ear Surgical Support (EESS) program. The planning and implementation of the CIEH program will be undertaken in partnership with eye health care stakeholders working in the jurisdiction, including Primary Health Networks, Local Hospital Networks/Districts, Aboriginal Community Controlled Health Services and their jurisdictional affiliates, eye health care service providers, state/territory health departments and local health organisations. The main objective of the grant opportunity is to improve access to eye care for First Nations people through improved coordination of culturally appropriate and safe services. The intended outcome of the grant opportunity is to improve integration of services from initial consultation to treatment, referral and continuity of care, to allow for streamlining and improving the care pathway for First Nations patients.
